Physician Referrals

If you are a physician or physician representative making a referral to University of Minnesota Physicians, please call the scheduling line at 612-626-8430. Please be prepared to provide the following information when you call to make an appointment:

• Patient's name
• Patient's telephone number
• Patient's date of birth and social security number
• A brief description of the patient's clinical condition
• Restrictions and/or requirements for the referral
• A time frame for when the appointment should be scheduled
